Registration for our fifth annual Transition Summit begins on March 1 at https://ctc2019summit.eventbrite.com

On the agenda …

Creating a Vision for the Future: Kelly Stumme and Caren Musembi, Ray Graham Association, will talk about what families should consider as they plan for the future and how various residential options can fit into that plan.

Supportive Housing: Julie Nelson, Corporation for Supportive Housing (CSH), will explain what Supportive Housing is, how to access it through the Statewide Referral Network, and then how to obtain the additional person-centered services needed for success using CILA, Home-Based, or private funding. We’ll also hear from David F. and Clearbrook’s Jennifer Robinson about David’s experiences finding his own apartment using the Supportive Housing model.

Community Integrated Living Arrangements (CILA): Usually thought of as community-based group homes, CILAs can take many forms. We’ll hear about several CILA models including traditional and family-owned, as well as options for those with higher medical or behavioral needs.

Family-Driven Creative Models: Two area nonprofits, Our House Communities (ourhousecommunities.org) and Project ALEX Communities, Inc. (projalex.org), will discuss their efforts to create new and different housing options for people with disabilities.

Registration fee of $20 includes morning refreshments, lunch, and seminar materials. You must register by Wednesday, April 10; space may be limited! Parents, educators, service providers and community members are welcome. CEUs/PDs may be requested at registration.

The Transition Summit is offered in partnership with the Hoffman Estates Commission for People with Disabilities and the Northwest Suburban Special Education Organization (NSSEO) and is paid for in part by a grant from Palatine Township. Thank you for your support!
